Our Administrator, Director of Nursing and a member of our Transitional Care Consultant team will connect with you and set-up the best time to visit with you and your loved ones.
Our goal is to visit you in person within 1-2 days to present our Transitional Care Plan options.
You choose which plan is best for you and your family.

Our team of Transitional Care Consultants will coordinate every step of your loved one’s transport to the comfort of your home.

To both prevent and manage COVID-19 related risks and symptoms, a specialized COVID-19 Transitional Care Team including a nurse, aide and volunteer dedicated specifically to the care of you and your loved one.
Full PPE (Personal Protective Equipment) including masks, gloves, gowns and goggles.
A COVID-19 Care Kit for you and your family.

Your Care Team Members Include

Each patient and family receives their own advanced home care support team.

Physician

Oversees your plan of care and advises your care team in ways we may best support you during this time.

Nurse

Visits weekly or more frequently as needed.

Nurse’s Aide

Gives personal care, such as bathing, dressing or assisting with light household chores.

Social Worker

Offers emotional support and helps with transitional care planning, such as living wills, funeral arrangements and community resources.

Chaplain

Facilitates conversations around transition and provides emotional and spiritual support as well as grief counselling.

Volunteer

Provides friendship and companionship. Implements opportunities to do things that you may enjoy from the comfort of your home each week.

Our No One Dies Alone Program brings specialized team members skilled in transitional care to support our patients and families as they are in their final 24-48 hours.

We sit bedside and hold the hand of our patients as we read poems and listen to their favorite music. We master the art of being present and wait by their side as our patient’s transition.
